Colleen has been an LPN with CDS Monarch for 16 years, starting out at Perinton and then moving to Blended Creative Opportunities when the Wolf Life Transitions Center opened in 2008. In February of this year she took over the nursing responsibilities at at Penbrooke and Linden Creative Opportunities. Colleen is responsible for attending individual's meetings, passing their medications, and taking care of them when they are sick or have concerns. She also does care followups with staff and families when individual's head home for the day, tracks medication and immunization orders and assists other nurses as needed. Previously, Colleen served on the Golf Committee and was awarded "Clinician of the Year" in 2007.

"Colleen has been an important asset to the Day Services setting for many years. This past year, when we needed help at LCO and PCO, Colleen stepped in with great gusto and enthusiasm. She knows how to organize, and got the nursing offices and records up to her high standards quickly and efficiently," stated Debbie Smith, Director of Clinical Services. "She really takes ownership of whatever site she is assigned to, and always has the well-being of the individuals first and foremost in her heart and mind."

"It is great to be a part of the individuals' family," says Colleen. "I feel valued and accepted by the individuals and their families and it is nice to know that I have their trust when it comes to making decisions about their care."

Colleen's advice for success? "Go with the evolution. Everytime something changes, CDS Monarch sets the bar higher, and it is always for the better."

In her free time Colleen enjoys spending time with her granddaughter, niece and husband. She also likes to cook and bake, sew, crochet and read. Thank you Colleen for your positive attitude and dedication to all of the individuals at CDS Monarch!
